What is the KPC Health Transfer Center

Thank you for considering KPC Health for your patient’s healthcare needs. Our Transfer Center provides a simple path for requests from outside facilities to refer their patients to one of our locations for quality, timely care.

If you are a physician, case manager, insurer, or hospital representative, you may reach out directly to our dedicated Transfer Center staff to initiate a patient referral. If you are a patient yourself or the family member of a patient seeking a transfer to a KPC Health facility, please reach out to your physician or case manager.

By working through the KPC Transfer center, we strive to make the transfer process as accessible and effective as possible for all healthcare professionals.

Initiating referrals through the KPC Transfer Center ensures a quick response to your transfer requests and expertly coordinates with the most appropriate hospital and physician tailored to your patient’s needs, all while keeping insurance and financial needs as a top priority. If express transfer services are needed outside of the operating hours for the KPC Transfer Center, each of our hospital’s skillful House Supervisors are prepared to facilitate all transfer requests
